This guide is provides a template for observing and recording student behaviors that may be indicators of possible ASD. It provides clear descriptions and specific examples of behaviors typically observed a classroom/school setting.

Parent Interview

This form provides a framework and specific prompts for conducting an interview for family members of a child who may have autism. It collect basic background, health history, and family concerns as well as more detailed information about the childs communication, social, and behavioral characterists as observed by the family.
